Recall Notification Acknowledgement

Please acknowledge that you received notice of this product recall by completing the “Recall Notification Acknowledgement” form.

Whether or not you have any of the affected product, you MUST acknowledge the receipt of the recall notice.

Recall Item(s) Photos

Please review the photos on this page, showing the product information and specific lot code(s) being recalled.

If you have affected product please select the “Recall Response Form” link and let us know how much product will need to be picked up.

Step 3:

Recall Response Form

Once the Recall Acknowledgement & the Recall Response Form are completed, a pickup will be generated. Upon receipt of product at our warehouse, credit is issued.

Please ensure you are ONLY sending back product that meets the specific lot code information and/or dates associated with the recall.
